I can't really base my reply on knowledge, merely observation. I've only just purchased my first winch! If you want out right balls to the wall grunt then the PTO is the answer. I had this issue on a honda as well, it felt like a shaking at a certain speed, if I increased / decreased is went away.
It turned out it was a drive shaft uni bearing or two...

Does it do it at idle or only driving?
Does it smoke less / more before problem started? I assume it's a diesel here

The truck must have had a larger one on it in the first place becasue the bolt holes don't line up.
