lack of maintenance and mud etc grinding it because of no grease.I would say .. grease it a lot if off-roading .. but don't over grease it otherwise it'll just blow to bits ( hydraulic ) Just a couple pumps or 3-4 will be enough

So will it need a new drive shaft or could it be fixed?

I'd suspect it is fkd now so new or good second hand one..to sloppy and it'll just pop out when you least expect it to.. and the damage is not worth that.. it'll fk ya trans, basically everything under next to it if you don't stop quick enough as it'll swing around bashing everything in it's path.. major then.. fix it now problem goes away

it'll be sloppy worn splines in the slip joint mate it's pretty common with worn out old driveshafts, it cant come apart unless it breaks in half :D you should be able to find a reasonable secondhand one considering its the front and shouldn't have as much wear but you might have to sift through a few to find a good one :D allfourx4 in Oz sell new toyo ondriveshafts :D
